GENERAL INFORMATION ABOUT FUNCTIONAL BEHAVIOR ASSESSMENTS
RECORD REVIEW FORMS:
Every FBA should start with a review of past information. These are some forms that can be used for that.

INTERVIEWS:
Interviews can be used as part of a functional assessment but are typically best used in combination with observational data.
Functional Assessment Checklist for Teachers and Staff (FACTS)—has directions and is aimed at efficiency. There is some evidence that shows it is effective when combined with a confirmatory observation.
